Community Nurses
Community Nurses provide a range of care services for patients in their own homes. These include,
- Assessment and Health Surveillance of older people with chronic illness
- Post Operative Care
- Wound Care
- Continence Service
- Care of the terminally ill
This team is based at Sevenoaks Hospital, but they come into the Sevenoaks Practice every day.

Health Visitors
Health Visitors are qualified nurses with specialist training and experience in child health promotion and family centred public health. Although concerned with all age groups, they have a special responsibility for the under-fives.
They offer advice on child health and development and provide support to families in the post-natal period. Health Visitors are also concerned with children with special needs, single parents, young people and families under stress.
The Health Visitors Team can advise you about the local baby clinics that you can attend.

Midwives
Once your pregnancy has been confirmed by your GP, your ante natal care will be provided by a team of midwives. They are based at The Pavilion in the grounds of Sevenoaks Hospital and provide most of the local care for pregnant women and also for women with newborn babies once you are home from hospital.
You will enter into a trusting relationship with your midwife who will get to know you and work with your family unit towards a satisfactory and fulfilling outcome to your pregnancy. The midwives run antenatal clinics at the hospital and throughout the week and are available to offer advice on pregnancy, labour and postnatal period.
