Our online form is for non-urgent medical questions and admin requests only. This includes things like:
- Routine medical advice
- Repeat prescriptions
- Test results
- Fit notes
- General admin enquiries
Please do not submit the form for urgent or sudden health problems — we cannot review these online. If you need same-day care for something serious, call us or come in.
How it works
- Complete the short online form with your details and question. Please only submit 1 problem per submission
- Our team reviews requests Monday–Friday, 8:00 am–6:30 pm.
- We aim to respond within 2 working days.
If you can’t use the internet or need help, you can call or visit the surgery — a Receptionist will help you.
When not to use this form
Do not use the online form for urgent or acute health issues such as:
- Severe chest pain or breathlessness
- Stroke signs (face drooping, speech problems, arm weakness)
- Heavy bleeding or serious injury
- Severe abdominal pain
- New confusion, collapse or seizures
- Acute infections (e.g. urine infections, chest/tonsil/ear/throat infections, new rashes, fevers)
- Severe or sudden pain
For these, call us directly or follow the steps below.
What to do instead
During practice hours
Call the surgery team — they can help you get seen or advise you.
Outside practice hours
Call NHS 111 for urgent medical advice.
Life-threatening emergencies
Call 999 or go to A&E immediately.
