We are able to arrange most vaccinations required for foreign travel. Please consult the practice sufficiently early.
We need at least six weeks prior to departure is required to allow an appropriate interval between certain vaccinations if needed, and ensure that you are offered appropriate advice. If you have had vaccinations done elsewhere (not at Amherst) please ensure you bring a record of these to ensure.
Please call the Reception to book an initial appointment with one of our travel nurses. At this appointment (one per traveller) you will need to complete and bring this Travel Checklist form with you. The forms are also available from reception.
One form needs to be completed for each patient due to GDPR (condifentiality). We are not able to process multiple family members via one form.
Cancel an appointment, enquire about prescriptions, change your address, send in a travel form, make a general enquiry, or advise of your next of kin.
Use Patient Access to order repeat prescriptions and view your medical record.
