Community Nurses
There is a team of Community Nurses and they are extensively trained in hospital before undertaking District Nurse Qualifications or Specialist Training.

These Community Nurses provide a range of care services for patients in their own homes. These include,

  • Assessment and Health Surveillance of older people with chronic illness
  • Post Operative Care
  • Wound Care
  • Continence Service
  • Care of the termally ill

This team is based at Sevenoaks Hospital, but they come into the Sevenoaks Practice every day.

If you wish to contact a member of the community team you can telephone the practice and speak to your Doctor's secretary who will pass on information for you or you may phone their Sevenoaks office
