Well Woman Life Style Review and the objectives
0ur bodies develop and change as we go though life and we need to understand this to ensure good health in the future.

Checks are held by the Practice Nurses and include Cervical Smears, Education about Breast Cancer and Guidance on Self-examination.

Other areas discussed include smoking, your weight and lifestyle, exercise, and being aware of the signs of the onset of a serious illness.

Our bodies develop and change as we go through life, but perhaps in a more gradual way than with women. This does not mean that a Well Man Check is any less important to ensure activity and good health in the future.

Checks are held by the Practice Nurses and are particularly directed at the prevention of Heart Disease, and the Control of Hypertension.

Other areas discussed include smoking, your weight and lifestyle, exercise and being aware of problems associated with ageing such as Prostate Gland. Awareness of the signs of the onset of a serious illness is also included.

      Baby and Maternity Clinics



Ante Natal Care

Once your pregnancy has been confirmed by your GP your care becomes shared with your GP and your midwife.

Antenatal care is provided by the midwives at THE PAVILLION in the grounds of Sevenoaks Hospital each midwife has their own clinic times.

You will be given a named midwife and the aim is to provide most of your care for the ante natal and postnatal period and in the first 10 days after your baby is born you will be visited at home but not every day. On some occasions another member of the team may also provide your care.

You will enter into a trusting relationship with your midwife who will get to know you and your family unit towards a satisfactory and fulfilling outcome to your pregnancy.

Non urgent messages can be left for the midwives at Sevenoaks Hospital on 01732 470341 or 570343 if at all possible please try to contact your midwife during her clinic time.

Ante Natal Care Timetable
Checks are carried out at certain time periods throughout your pregnancy starting with a booking which is usually carried out at home.

During your antenatal care you will also be introduced to your Health Visitor who will meet with you before you have your baby and in your early days at home with the baby.

The Sevenoaks Team of Midwives will offer Parent Craft Classes and this will also include a tour of Pembury Hospital. The classes are held at Sevenoaks Hospital usually on a Wednesday evening.
Aqua natal and yoga classes are held separately within the area.


The Plan for your antenatal care is as follows but this is a guide only.
When you discover you are pregnant you visit your GP who will refer you to your midwife.

You will receive some information from the midwife, which will help you to decide where to deliver your baby as geographically you have several hospitals to choose from.

If you choose to book at Pembury your booking visits will be at your home and you will be cared for by the Sevenoaks Team of Midwives,

If you choose to book at another hospital you will be booked by your chosen hospital and then cared for by the Sevenaoks Team Midwives.

If you have seen the GP very early on in your pregnancy you will not hear from the midwife before 8 weeks.

811 weeks A midwife will contact you and arrange your booking appointment at your home.
16 weeks See your midwife and collect blood results.
20–22 weeks Anomoly Scan.
24–25 weeks See your midwife.
28 weeks See your midwife who will take more blood.
30–31 weeks See your midwife and collect blood results.
36 weeks See your midwife.
38 weeks See your midwife.
40 weeks See your midwife, at this appointment your 41 week appointment will be arranged.
41 weeks Membrane sweep and induction date booked.

It is not always necessary if it is not your first pregnancy to see your midwife at 25 and 31 or even 40 weeks unless any concerns arising at booking or at the 20 week anomaly scan.

We regularly supply the immunisations needed for complex itineraries and use our modern IT systems to access the most up to date information as required. In addition to this they are in contact with specialist travel centres and the companies producing the necessary vaccines.

      Special Clinics



Coronary Heart Disease
This clinic offers annual appointments with our CHD trained nurse for monitoring of Blood Pressure, Cholesterol and medications. This allows the patient the opportunity for questions and advice on any areas of concern such as diet and exercise. We aim to reduce the risk of future heart related illness.

Hypertension Monitoring
If you are receiving treatment for High Blood Pressure it is important to have regular checks. Our aim is for patients to have a BP of 140/90 or below.

High Blood Pressure can cause strain on the vessels carrying blood around the body, which may eventually lead to Heart Attacks and Strokes.

We will carry out Lifestyle checks, weight and exercise and discuss Health Eating. We will answer questions and give encouragement where it is needed.

Additional support is available for monitoring your own blood pressure over an extended period. There are two documents that support this, and they can be downloaded as required.

Respiratory Clinics
Our nurse led ASTHMA and COPD clinics offer an opportunity for review and discussion. We aim to work with you to find the most appropriate treatment leading to optimum symptom control.
We also have up to date equipment allowing us to offer the 2 yearly lung function testing required for COPD patients.
Annual review is recommended and appointments for both clinics can be booked via reception.

Stroke Monitoring
If you have had a stroke it is important for you to attend our clinic for Stroke Review. This is an opportunity for the lead Nurse to check that you are receiving all appropriate medications and treatments, and you will be offered advice to protect you as far as possible from further problems.

Details of other clinics for the treatment of Diabetes, Leg Ulcers etc can be obtained from reception.

Morning After Contraception
Any woman over the age of 16 can go to a Local Pharmacy and request "morning after" contraception advice.
Treatment may cost in the region of £20.


Conception
Conception advice or any problems associated with conception are normally discussed and dealt with during an appointment with a doctor.

Minor Operations can be done by a Doctor
Minor Operations can be done by a Doctor at the Local Facility at Sevenoaks Hospital.
Examples of Minor Operations are:-

• Excision of lumps and bumps.
• Cautery of skin leasons.
• Biopsies.

This represents a fast and easy way of dealing with this type of problem.

      Medical Examinations, both NHS and Non NHS



Routine Medical Examinations
We offer Routine Medical Examinations because
we are committed to health promotion for all our patients.

If you are aged 15 – 74, you can come every 3 years for an brief health check. This will usually be done by one of our Practice Nurses or possibly your doctor if you have a chronic condition.

Your weight, height and blood pressure will be recorded and a urine sample tested.
We will discuss smoking, diet, exercise, alcohol intake, and appropriate advice may be given.

Please note that this a brief basic health check; however it gives us the opportunity to prevent many problems before they arise.

If you are aged 75 or over we can offer an annual health check with one of our nurses.

In addition to the Routine Medical Examination we will assess functions such as mobility, hearing and vision. This will be done by either a doctor or one of our nurses. If you are mobile do come to the practice for this check.

Commercial and Insurance related Medical Examinations
Many Organisations now require a Medical Certificate for Insurance, Occupation or Work. These Non NHS Examinations will be completed by a Doctor and charged at the Practice Rate.

Please call Practice Reception to be advised what is required.
Please make it clear that this is a commercial transaction and find out what cost are involved.
You will be required to pay at the time of your visit to the Practice.

      Private and Professional Services - List of Fees



Private and Professional Fees
for non-NHS Clinical Services and Administration

The Practice is often required to perform tasks that are not covered by the NHS
.

Tasks not covered by the NHS can include non health related blood tests and any clinical information or assessment made for commercial reasons
. These can typically include information required for driving licences, insurance or by an employer.

A list of fees for services available to Private Patients is also included.

Private and Professional Services - List of Fees

If you require your Doctor to complete forms, applications, letter to a third party, or other items of administration it is essential that you allow up to 7 working days for the paperwork to be completed.

The volume of paperwork that GPs are increasingly required to deal with takes considerable time and usually involves secretarial support.

We will always do our best to assist you if your need is very urgent. We do try to complete the work within five working days, but this can never be guaranteed.

If your Doctor is away from the Practice for any reason the receptionist will inform you that additional time may be required.
